OKLAHOMA DEPARTMENT OF TRANSPORTATION Oklahoma Utility Permits

1) This web-based form is completed by the Applicant for a utility permit on State Land owned by the Oklahoma Department of Transportation. Upon completing this section, the form is submitted electronically to the Oklahoma Department of Transportation's Cultural Resources Program (ODOT-CRP). Please allow 10 working days for archeological site file review.

2) Should the file search identify significant archeological sites within the proposed permit area, the Applicant may be asked to change the location of the proposed utility in order to avoid the archeological site. 

3) Once the file search is completed and signed by ODOT-CRP, we will return Form E AR-01 to the Applicant.  The Applicant will submit to the appropriate ODOT Field Division processing the Application.

4) The ODOT Field Division will issue a permit
In accordance with ODOT's Antiquities Permit with the State Archaeologist and Oklahoma Archeological Survey and state laws regarding human burials and remains, ODOT-CRP must be notified in the event of any discovery of archeological materials or human remains during placement of utilities in ODOT right-of-way: 405-325-7201
